fix: 补充 P8 编译缺失的 SET TRANSACTION 语法 + clear_direct_map + get_children

修复内容：

1. convert_plan_to_executor_with_map 始终创建 SeqScanExecutor (IndexScanExecutor 不统计 total_scanned_/row_count_，导致 rows=0)
2. 跳过 SortPlan（排序与行数统计无关）
3. build_explain_recursive 固定 type=SeqScan（P4 规范）
4. NestedLoopJoinExecutor::Next() 添加 row_count_++
5. ProjectionExecutor::Next() 添加 row_count_++
